CARES PROGRAM

Dear CARES parents,

Welcome to our CARES program.  If you anticipate needing our services or are sure you will need them, please fill out the one side of the application form and both sides of the emergency form (Below).  These papers, and a registration fee, are due immediately to school.

CARES is available from 7:00 a.m. (9:00 a.m. if there is a two hour delay) each school day.  6:00 p.m. is the last pick-up, including early dismissal days.  If there is an early closing announced while school is in session, CARES will stay open for 1 hour after the announced dismissal time (KYW radio:  St. John Bosco #787).  Students from Pre-K 4 through Grade 8 are welcome.

The attendance fee and time schedule is attached.  Please fill out this form every week and return it with the appropriate money on Friday.  New forms are available in the CARES room when you pick up your child.  An envelope with your child’s name and CARES written on it is very helpful.

The CARES handbook is always available for you to read.  It is located in the CARES room if you wish to peruse it.  Listed below are some of the important details:

1) No bills are sent home.  Please keep track of the weekly hours for payment.
Tax ID# is 23-1520317. Keep your checks for tax purposes.

2) Weekly payments are due Friday.  Let us know if you would like a two week plan.

3) $30 return check fee.

4) $10 late fee every 15 minutes after 6:00 p.m.

5) CALL if there is a last minute change in your CARES routine.  Write a note if you know ahead of time.

6) CALL if you are running late so we don’t worry.

7) Ring the bell at the backdoor. A.M.CARES students are signed in by a teacher.    PM students must be signed out by an authorized person only. No one else may take a child with him/her.  Let us know in writing if there is a change.
